To change your name, you will need to complete and submit a Name Change Request and Attestation Form along with a copy of the legal document changing your name to nsbla.office@nebraska.gov or by mail. Neb. Rev. Stat. Sections ยงยง 4-108 through 4-114 require all applicants for public benefits in Nebraska, including professional licensure, to provide documentation that they may be in the United States legally. The Board uses your documentation to verify your alien status with the US Department of Homeland Security. The application process cannot be completed until the lawful presence is verified.

The Board will not be able to process your application until your alien status can be verified with DHS. If we are unable to verify your status, you will receive notice from the Board that you will need to contact USCIS to update your status.

You must have a CLARB record for initial licensure(Chapter 2 of Title 231 NAC).

Comity applicants only need to provide verification of licensure in good standing in a jurisdiction recognized by the Board, directly from the issuing jurisdiction, to the Nebraska Board.

In most cases, licenses may be administratively issued by staff on a conditional basis once all supporting documents have been received. Formal approval will occur at the next scheduled regular board meeting. The licensee may start practicing upon issuance of the conditional license.

The exceptions are applications that have disclosures as outlined in Board Rule 1.4.5 and reinstatement applications. These must go to the Board for approval before the license is issued or reinstated.

To the meeting schedule, please visit our Meeting Information page.
